And if it is more plausible of a friend to do well by another person or man than to be well done by, and to bestow rewards is a characteristic of the good man and of virtue and it is rational and nobler to do well by friends than by unknown individuals or strangers. Therefore, the good man will need people for them to prosper and do well in their lives. This situation propels us to ask whethe r we need, friends in adversity or for success/prosperity, on the assumption that a man in adversity requires people to give rewards on him, and those who are successful need people to be more successful than they are. Endometrial Cancer Essay Endometrial cancer refers to several types of malignancy which arise from the endometrium, or lining of the uterus. Endometrial cancers are the most common gynecologic cancers in the United States, with over 35,000 women diagnosed each year in the U. S. The most common subtype, endometrioid adenocarcinoma, typically occurs within a few decades of menopause, is associated with excessive estrogen exposure, often develops in the setting of endometrial hyperplasia, and presents most often with vaginal bleeding. Endometrial carcinoma is the third most common cause of gynecologic cancer death (behind ovarian and cervical cancer). A total abdominal hysterectomy (surgical removal of the uterus) with bilateral salpingo-oophorectomy is the most common therapeutic approach. Endometrial cancer may sometimes be referred to as uterine cancer. However, different cancers may develop not only from the endometrium itself but also from other tissues of the uterus, including cervical cancer, sarcoma of the myometrium, and trophoblastic disease. Classification Most endometrial cancers are carcinomas (usually adenocarcinomas), meaning that they originate from the single layer of epithelial cells which line the endometrium and form the endometrial glands. There are many microscopic subtypes of endometrial carcinoma, including the common endometrioid type, in which the cancer cells grow in patterns reminiscent of normal endometrium, and the far more aggressive uterine papillary serous carcinoma|papillary serous carcinoma and clear cell endometrial carcinomas. Some authorities have proposed that endometrial carcinomas be classified into two pathogenetic groups:[1] â⬠¢ Type I: These cancers occur most commonly in pre- and peri-menopausal women, often with a history of unopposed estrogen exposure and/or endometrial hyperplasia. They are often minimally invasive into the underlying uterine wall, are of the low-grade endometrioid type, and carry a good prognosis. â⬠¢ Type II: These cancers occur in older, post-menopausal women, are more common in African-Americans, are not associated with increased exposure to estrogen, and carry a poorer prognosis. They include: â⬠¢ the high-grade endometrioid cancer, â⬠¢ the uterine papillary serous carcinoma, â⬠¢ the uterine clear cell carcinoma. In contrast to endometrial carcinomas, the uncommon endometrial stromal sarcomas are cancers which originate in the non-glandular connective tissue of the endometrium. Uterine carcinosarcoma, formerly called Malignant mixed mullerian tumor, is a rare uterine cancer which contains cancerous cells of both glandular and sarcomatous appearance in this case, the cell of origin is unknown. Signs Symptoms Vaginal bleeding and/or spotting in postmenopausal women â⬠¢ Abnormal uterine bleeding, abnormal menstrual periods â⬠¢ Bleeding between normal periods in premenopausal women in women older than 40: extremely long, heavy, or frequent episodes of bleeding (may indicate premalignant changes) Women with stage 1 disease who are at increased risk for recurrence and those with stage 2 disease are often offered surgery in combination with radiation therapy. Chemotherapy may be considered in some cases, especially for those with stage 3 and 4 disease. hormonal therapy with progestins and antiestrogens has been used for the treatment of endometrial stromal sarcomas. The antibody Herceptin, which is used to treat breast cancers that overexpress the HER2/neu protein, has been tried with some success in a phase II trial in women with uterine papillary serous carcinomas that overexpress HER2/neu. Radiation therapy for endometrial cancer Radiation therapy is the use of high-dose X-rays to destroy cancer cells. Radiation therapy may be used to treat endometrial cancer after hysterectomy or as the primary therapy, particularly for women who cannot have surgery. The two types of radiation therapy that may be used to treat endometrial cancer are: â⬠¢ Internal radiation therapy (brachytherapy), in which radioactive materials (radioisotopes) are placed into the uterus or other areas where the cancer cells are found. External beam radiation therapy, in which radiation comes from a machine outside the body. In brachytherapy, tiny tubes of radioactive material are inserted through the vagina and left in the uterus for 2 to 3 days. Brachytherapy may be done in the hospital or as on an outpatient basis. External beam radiation therapy is usually done in an outpatient treatment center. A typical therapy course is radiation treatments 5 days a week for 4 to 6 weeks. Radiation therapy may cause side effects, including: â⬠¢ Fatigue. â⬠¢ Dryness, itching, tightening, and burning in the vagina. Red, dry, tender, itchy skin. â⬠¢ Moist, weepy skin (later in the treatment). â⬠¢ Hair loss in the treated area. â⬠¢ Loss of appetite. â⬠¢ Diarrhea. â⬠¢ Frequent and uncomfortable urination. Before formulating a strategy, an organization has to gauge its current position in the market using strategic analysis. This involves the use of internal and external analysis tools to gain both an inside view of an organization and the macro environment. Internal analysis tools are used to identify and evaluate an organizationââ¬â¢s strengths and weaknesses in terms of its resources, operational capabilities and core competencies. This gives the organization a picture of what strengths to exploit and develop further, and what weaknesses should be corrected to reduce market liability (Hill and Jones 2012). External analysis tools such as Porterââ¬â¢s five forces gives a view of the immediate competitive environment to reveal market opportunities and threats. It allows the organization to identify the market forces, which they have little to no control over, in order to develop contingencies into their strategic plans (Porter 1980). Dynamic capabilities such as SWOT analysis, is a combination of internal and external analysis to reflect an organizationââ¬â¢s ability to adapt to volatile markets (Teece 2009). The strategic analysis process would provide a comprehensive overview of an organizationââ¬â¢s competency, which then provides the basis for strategic formulation. A suitable approach to strategy formulation would strongly depend on the size and nature of the organization. One method that can be applied across different industries as well as organizations sizes is Porterââ¬â¢s generic strategy. It involves three different strategy classification; low cost leadership, product differentiation and target market focus (Porter 1980). Another method is Miles and Snowââ¬â¢s (1978) typology. It suggests that competing organizations are characterized based on their individual view of the competitive environment and how they allocate resources accordingly. The four basic categories are defender, prospector, analyzer, and reactor. Formulated strategies are not always acted upon, but may serve as a learning process towards more efficient strategy formulation. Allio (2005) states ââ¬Å"immediately following the formal ratification of the firmââ¬â¢s vision and set of strategies, implementation can begin in earnestâ⬠. However, implementation of a strategy is considered the greatest challenge due to the risk of setbacks such as the lack of communication throughout the organization, unclear planning as well as poor monitoring and controls. The main purpose of the impact evaluation is to equally evaluate Enterprise System (ES) impact and Competing Value Approach (Leon, 2012). Evaluation of the Heart and Stroke Foundation of Canada Research Scholarship Program: research productivity and impact. The researcher make use of impact and outcome evaluation to determine whether funded or unfunded individuals can be more productive and have greater body knowledge in heart and stroke area. The study consist of the funding years 1980/81 to 1989/90 inclusive, number of peer reviewed publications and data were collected on 192 individuals to determine a result (Armstrong, 1997). The main purpose of this study is evaluate both funded and unfunded individuals to see which individuals has more impact on the knowledge and productivity (Armstrong, 1997). The outcome of this study shows that funded individuals has more impact on the knowledge and are more productive than individuals that are not funded. Why I think the researcher use this type of evaluation is to determine the unequivocally relation between funding and research success, it indicate a suggestive beneficial effect (Armstrong, 1997). Current status of acute stroke management in Korea: a report on a multicenter, comprehensive acute stroke registry. The researcher make use of input and outcome evaluation to determine the care of stroke patients in Korea at national level. An evaluation of stroke rehabilitation within Greater Manchester The researcher make use of impact and outcome evaluation to determine the rehabilitation services and evaluate the stakeholders opinion. The study analysis 214 separate recommendations from 15 documents. Of these are 21 were relevant to every patient receiving stroke rehabilitation, 13 to overall service provision and 8 related to specific aspects of patient care. These were converted to standards and used to audit the 10 stroke rehabilitation services in Greater Manchester. Patients, staffs and commissioners completed questionnaires to determine the national recommendations of services (Alison, 2013). The main purpose of this study is to evaluate the feeling of stroke rehabilitation for patients and what improvement can be taken (Alison, 2013). Study shows information that will describe the feelings of patients towards rehabilitation, improvements can be done in future studies and reason why I think the researcher use this evaluation is to create future studies and to improve more towards rehabilitation experience, giving satisfaction to patients (Alison, 2013). Evaluation Model PRECEDE-PROCEED Evaluation Model The Predisposing, Reinforcing and Enabling Constructs in Educational Diagnosis and Evaluation and Policy, Regulatory, and Organizational Constructs in Educational and Environmental Development (Precede-Proceed) evaluation model is designed for cost-benefit evaluation framework. To help health program planner, policy makers and other evaluators to analyze and design health programs systematic. It gives a comprehensive structure to asses health and quality of life and to design, apply and evaluate health promotions and public health programs. The systematic use of the evaluative model uses a series of clinical and field trials to confirm the advantage and predictive of the model. The main purpose of this evaluation model is to directly put attention to outcomes rather than inputs. It allows planners to start with the desired outcome then work backwards to identify strategies to achieve those objectives (Andrea, Gielen, Tiffany, and Gary, 2017). The PRECEDE-PROCEED model has been a utility program for practice settings and researchers conducting health changes (Lawrence, and Judith, 2006). The context evaluation main purpose is to evaluate the growing recognition of the health education to enclose policy, regulatory and related ecological/environmental factors, determining health and health behaviors, to improve and to increase scholarly productivity among health education faculty. Furthermore, the evaluative model is not to predict or explain the relationship among factors thought to be associated with an outcome of interest. Rather, the main purpose of it is to provide a structure for applying theories and concepts systematically for planning and evaluating health behavior change programs (Lawrence, and Judith, 2006). This evaluation model may contribute for CVA all around the world, especially for Ministry of Health of New Zealand. It can improve its study target starting from a diverse array of assessment such as social, behavioral, educational and existing policies. Furthermore, auditing the CVA cases can be implemented through predisposing, reinforcing and enabling the studies. Eventually, cases will be evaluated to enhance the quality of life (Lawrence, and Judith, 2006). Conclusion Evaluation research is needed in all aspects of healthcare implementing programs or project to improve an overall effectiveness. It is very important to use the right tools, followed by numbers of reviews and research towards the improvement or success of the organization. This involves education, managers and staffs feedback can develop a thorough guidelines to help improve their systems and policies. Overall, the evaluation research can improve the common good and development of the organization and community can benefit from the outcome and achieving objectives in most efficient ways.
